Booking Form
The Booking Form should be completed to arrange a Combat Zone event.

Please note - First time bookings are encouraged to download the Organiser's Pack for complete booking instructions.

Credit Card Authorisation Form
The Credit Card Authorisation Form should be completed when you wish to pay via for your Combat Zone event by credit card.

Indemnity Form
The Indemnity Form must be completed, signed and handed to the game administrator prior to game play.

Please note - All participants under the age of 18 must have the form signed by a parent/guardian.

Organiser's Pack
The Organiser's Pack contains all information to assist you in organising a Combat Zone event.

Please note - Organiser's Pack contains the Booking Form, Indemnity Form and Credit Card Authorisation forms

Invitation
Download and fill in the Invitation to send to your friends for your event.

The invitation makes use of PDF form fields. Enter text on the form then print out. (2 invitations per A4)
